What to Expect at Your First Visit

What to Expect at Your First Visit

During your initial appointment, you will meet with one of our orthopedic providers who will evaluate your condition and discuss treatment options.

Your visit may include:

  • A review of your medical history & symptoms

  • A physical examination

  • In-office X-rays, if needed

  • Discussion of prior imaging (MRI, CT Scan, or X-rays if available)

  • A personalized treatment plan

Our goal is to provide a clear diagnosis and outline both surgical and non-surgical treatment options when appropriate.

Please Arrive Early

We ask all new patients to arrive 20–30 minutes before their scheduled appointment time to complete any necessary paperwork and check-in procedures.

New Patient Forms

Save time at your visit by filling out these forms in advance.

What to Bring to Your Appointment

Please bring the following items to your visit:

  • A valid photo ID

  • Your insurance card

  • Any required referral or authorization (if applicable)

If you are unable to bring imaging with you, our office is able to obtain X-rays during your visit when appropriate.

Insurance & Referral Requirements

Insurance & Referral Requirements

HMO Insurance Plans

Patients with HMO insurance plans are required to obtain a referral and any necessary authorization from their Primary Care Physician (PCP) prior to their appointment.

It is the patient’s responsibility to ensure that all required referrals and authorizations are in place before being seen.

Failure to obtain proper authorization may result in rescheduling or financial responsibility for services provided.

PPO Insurance Plans

Most PPO insurance plans do not require a referral to see an orthopedic specialist. However, we recommend verifying your specific plan requirements with your insurance provider prior to your visit.
